Available for Groups and Individuals

Plan Design Benefit Highlights:

See Any Dentist Plan: (over 70 plans to select from)

• Preventive services (cleanings and x rays) normally covered at 100%
• Basic services (fillings and simple extractions) normally covered at 80%
• Endodontics (root canals) and Periodontics (gum disease) services available as a Basic service covered at 80%
• Major Services (crowns and dentures) normally covered at 50%
• Lifetime or Calendar year Deductibles are available
• $750 to $2,000 Calendar year Maximum’s are available
• Orthodontia (braces) services for children available as an option
• Implant benefits (Cosmetic Dentistry) are available as an option or included at no cost depending on the carrier
• Limited waiting periods on voluntary plans only (even for new hires).  Takeover benefits are available for employers who have a current dental plan in place and qualify
• Claims paid at either the 80^th or 90^th percentile…..you decide

Network Dental Plans: (Two plans to select from)

• No waiting periods on benefits (immediate coverage on all dental procedures)
• Unlimited benefits each year (use as much as you like)
• Orthodontics (braces) are covered immediately for Children and Adults
• No deductibles to satisfy
• No age Limits
• Cosmetic dentistry (implants, veneer bonding, bleaching) services available
• Select from over 600 credentialed dentists statewide
• Low monthly preferred pricing and rate guarantees

The Affinity Plan also allows members to enroll online in our Individual Dental Plan with preferred pricing and rate guarantees. The Alpha Dental Individual Affinity Plan providers are fully credentialed and are contractually required to offer excellent quality and service to all of our members.

Plan Highlights are:

• No waiting periods on benefits (immediate coverage on all dental procedures)
• Unlimited benefits each year (use as much as you like)
• Orthodontics (braces) are covered immediately for Children and Adults
• No deductibles to satisfy
• No age Limits
